I understand that falsification or significant omissions of any information may be reason to end my volunteer relationship with HRC. My acknowledgement below also authorizes educational institutions, corporations, companies, partnerships, persons, and city, county and federal agencies to release information relating to the position for which I wish to serve for HRC upon their request. A copy of this acknowledged form shall act as the original and have the same validity. I agree I do not agree The Hays Recreation Commission conducts background checks on all individuals who are employed by and/or volunteer for the HRC. This includes, but is not limited to: criminal background records/information, criminal background checks, driver's license checks, first aid experience checks, and address checks. All HRC employees and volunteers over the age of 18 including full-time, part-time, seasonal, and volunteers are required to adhere to a background screening. These screenings will be required every two (2) years. This must happen before starting work and/or volunteering. Background screenings are done online, please provide your email address to send the link to complete the screening if you are hired or used in a volunteer position. Thank you!
